Maintaining Access to Health Care into Adulthood

Planning for health care transition can be challenging, but it is necessary for all youth to maintain access to needed care. For many youth, the focus is on increasing independence, learning about their health needs, and using the health care system to meet them. For youth who require ongoing support, the focus is often on teaching another caregiver about the health needs and supports needed. The transition to adult providers also occurs during this time for all youth.

  1. Get started Learn about health care transition. Review the “Health Care Independence: It’s a Process” resource below.
  2. Assess where you are at Take a readiness assessment, such as the Transition Readiness Assessment (TRAQ) or one available from Got Transition.
  3. Choose a skill to build Review the assessment. What area of skill building or interest is identified?
  4. Set a goal Identify a goal to work on. Use the Taking Control of Your Health prompt cards and goal setting form to help.
  5. Use your tools and practice Use tools to support learning and practice. Try different tools and methods until you find what works for you.
