Planning for health care transition can be challenging, but it is necessary for all youth to maintain access to needed care. For many youth, the focus is on increasing independence, learning about their health needs, and using the health care system to meet them. For youth who require ongoing support, the focus is often on teaching another caregiver about the health needs and supports needed. The transition to adult providers also occurs during this time for all youth.
It can feel overwhelming and a bit scary. We are here to help. Use the resources on this page to get started. If you need support, our staff is available to help you at 603-271-4525.
