The Council for Youths with Chronic Conditions has partnered with the Community Health Institute/JSI (CHI/JSI) to conduct a needs assessment around children with chronic physical health conditions and their families.
CHI/JSI will be rolling out a statewide survey as part of this needs assessment and we want to hear from families about what sort of questions we should be asking to truly understand the successes and challenges youth with chronic physical health conditions and their families face in everyday life.
We invite parents/caregivers living in NH with a child 21 or under with chronic physical health conditions to join us for an informal listening session Wednesday, January 5th, 2022 from 5:30-6:30 pm via Zoom. Examples of chronic physical health conditions include but are not limited to asthma, epilepsy, cancer, diabetes, hemophilia, and neuromotor disorders such as spina bifida and muscular dystrophy.
