Community Health Care Programs for Children

The Child Health Program

Through this program, DHHS and the Bureau of Maternal and Child Health fund several community health centers and health agencies that provide healthcare services to children in their community. Services are targeted to low-income, uninsured, or underinsured children. Care is provided through office and home visits and is free or available by a sliding fee scale, depending on the child or family’s financial situation.

Services include:

  • Helping parents enroll a child on NH Medicaid if the child is eligible
  • Physical exams
  • Health screenings
  • Immunizations
  • Providing information on topics such as a child’s growth, development, and safety
  • Social services and
  • Helping parents coordinate care and get other needed services.

Child and Family Health Support Services

Community health agencies in the state receive funding from DHHS/MCH to provide “Child and Family Health Support Services.” These services help families where children may already have a healthcare provider but need additional services. They can also provide services such as:

  • Helping parents enroll a child on NH Medicaid if the child is eligible
  • Helping parents with coordinating care and getting other needed services
  • Providing information on topics such as a child’s growth, development, and safety
  • If the child does not have a health care provider (or “medical home”) or dentist, parents can get help finding one.
  • These services are conducted in person or by telephone.
  • These services are used primarily by children from birth through age ten but are available for children up to age 19.
