Council for Youths with Chronic Conditions

The Council for Children and Adolescents with Chronic Health Conditions analyzes the barriers for families of children and adolescents with chronic health conditions in obtaining appropriate and effective community and family-based services and support. The Council works with state and local agencies to improve communities’ capacities to respond to the needs of children and adolescents with chronic health conditions and provide support to their families.

We can have upwards of 13 parents sitting on our council as well as a youth member and a person experiencing a chronic health condition. A person representing a professional or community organization is also eligible.
